An allergy log template is a document which is used to confirm all the different kinds of allergies that an individual has and their adverse reactions to exposure to respective allergens. At the very top of the paper, the individual/patient is given a few blanks to fill in about their personal information. It usually includes the name, age, and contact information of the individual. In some cases, the allergy log asks the allergy-prone individual to state the time, date, occurrence and other details about their allergies in a table format. Most of the student allergy logs in schools also contain a column for the blood pressure log, which they should provide in case of an allergic reaction.
